Position Overview

The Iowa City Veterans Affairs Health Care System seeks a Full-Time Advanced Practice Nurse for Home Based Primary Care within the Rehabilitation and Extended Care Service Line. This role focuses on managing a panel of patients with acute and chronic conditions through home visits, including assisted living facilities.

Key Responsibilities

• Independently manage patient panels emphasizing health promotion and disease prevention
• Develop individualized care plans for optimal outcomes
• Order and interpret labs, tests, and diagnostics
• Prescribe pharmacologic and non-pharmacologic interventions
• Provide patient and family education
• Collaborate with interdisciplinary teams

Schedule

Monday through Friday, 7:30am-4:00pm. Alternative hours negotiable at hire. No telework or virtual options.

Compensation & Benefits

Salary range $117,200 - $169,900 per year. Eligible for Education Debt Reduction Program (EDRP) with up to $200,000 in student loan repayment over one to five years. Comprehensive VA total rewards package available.

Qualifications

U.S. citizenship required. English proficiency, background investigation, and pre-employment physical required. Seasonal influenza vaccination mandatory.
